I agree that all information answered on this form is accurate to the best of my knowledge. Candidates must be able to provide written proof of training or experience and documentation of education completion upon request. Candidates are solely responsible for knowing and understanding state regulatory requirements that may pertain to their certification and ability to practice in their profession in their particular state.
Your Phlebotomy Instructor Certification, like many credentials, will be active for the term of your certification. Phlebotomy Instructor Certification does not guarantee any candidate’s ability to meet any or all regulatory requirements about their profession. You must complete the CPT Certification every two years to keep your certification valid.
I understand that it is my responsibility to call the state before getting the certification to verify current requirements. Dependent solely upon your employer, as these two states have created state-specific licenses that may be required of you to perform Phlebotomy.
Please check with your employer as to whether these restrictions may apply to you before registering with Phlebotomy Instructor(PI). PI will not be held responsible if you fail to do so, and all applicable cancellation policies and fees will be applied. Please read through our Terms and Conditions before submitting your registration. I understand that all assessment questions are the copyrighted property of PI.
The removal or attempt to remove questions or other assessment material from the test site is prohibited. It is forbidden under federal copyright law to copy, reproduce, record, distribute, or disclose these assessment questions by any means, in whole or in part. A violation of this type can result in civil and criminal penalties. I certify that I will not cheat or violate the confidentiality of the examination.
Cheating or violation of confidentiality may be defined as, but not necessarily limited to the following:
a. obtaining help from any other person during the examination,
b. communicating with or giving help to another candidate during the examination,
c. using notes, books, or any other sources of information during the examination,
d. using electronic programmable devices, such as calculators, cell phones, and PDAs during the examination,
e.reproducing or making copies of an PI examination or test questions by any means,
f. memorizing and reproducing test questions, soliciting test questions from previous candidates,
h.discussing or disclosing the contents of the examination by any means, or
i. providing false or purposely misleading information when applying for, registering for, or completing the exam.
I understand that PI will publish a list of Certified Candidates and that my employer and state regulatory board may be notified if my certification expires or is revoked. Unless required by law, my name will not be released to any party in conjunction with my test score without my permission. I understand that any claim I may have related to the good-faith enforcement of these policies or the unintentional damage or loss of my exam records will not exceed the amount of my application fee for this examination.
Payment Plan Terms
This is a new program to Phlebotomy Instructor and we reserve the right to change, cancel, hold or suspend preliminary certification program at any time. By filling out and signing forms below, you commit to a subscription plan for the duration of your certification. Instead of a large upfront cost, we broke the cost down or one and two-year certification programs into affordable monthly installments with payment processor fees included in the program. You start with a two-month payment and then pay as you go, payment plan fees are non-refundable. Failure to pay for the first month will place your certification on hold and notify you of your certification status. A chargeback or subsequent failure will result in the suspension of certification, ban from payment program and a notice of failure to all entities who have inquired of your certification status as well as blacklisting you as an individual with national phlebotomy directory. We are taking a financial risk by offering this program and sincerely hope that this program helps a lot of people!

DETAILED TERMS AND CONDITIONS AND REFUND POLICIES
I at this moment, give my authorization to the Phlebotomy Instructor Corp for Healthcare Professionals to request necessary information from individuals, institutions, and organizations named herein to validate data for certification. I certify that the information given herein is true and correct, to my knowledge and belief, and realize that accreditation is subject to revocation for misrepresentation. If accepted as a phlebotomy instructor, I agree to uphold and abide by the Standards of Practice and Bylaws of the Phlebotomy Instructor Corp. I hereby authorize Phlebotomy Instructor Corp to investigate my background and qualifications for purposes of evaluating whether I am qualified for the certification for which I am applying. I understand that Phlebotomy Instructor Corp will utilize an outside firm or firms to assist it in checking such information, and I specifically authorize such an investigation by information services and outside entities of the company’s choice. I hereby consent to submit to a drug or alcohol test and to furnish a sample of my urine, breath, and/or blood for analysis, as shall be determined Phlebotomy Instructor Corp in order to meet with their policy regarding the selection of applicants for employment.
I further authorize and give full permission to have the Phlebotomy Instructor Corp and/or its authorized agents and physicians to send the specimen or specimens so collected to a laboratory for a screening test for the presence of any prohibited substances under the policy, and for the laboratory or other testing facility to release any and all documentation relating to such test to the Phlebotomy Instructor Corp. I further agree to and hereby authorize the release of the results of said tests to the PhlebotomyInstructor.com. I also agree to hold harmless PhlebotomyInstructor.com and its agents and physicians from any liability arising in whole or part, out of the collection of specimens, testing, and use of the information from said testing in connection with the Phlebotomy Instructor Corp consideration of my application of employment. The syllabus and material do not establish a contractual relationship between Phlebotomy Instructor Corp and/or any phlebotomy instructor-certification member. Rather the syllabus and education materials are provided to define a broad outline of the phlebotomy course structure and goals.
REVISIONS
The phlebotomy instructor reserves the right to modify, change, or otherwise alter the details of instruction and material coverage offered in any class in response to class progress and interests. Please remember that existing artwork, images, and text that you may want to include in your projects may be protected under copyright law. The unauthorized incorporation of such material into your new work could be a violation of the rights of the copyright owner. Please be sure to obtain all permissions required from the copyright owners. No part of this web site may be reproduced, stored in a retrieval system, or transmitted, in any form or by any means, electronic, mechanical, recording, or otherwise, without the prior written permission of the instructor.
MATERIALS AND RESPONSONBILITIES
The material on this website is reserved solely for use by the Phlebotomy Instructor Corp, and only those students, while enrolled in the specific courses listed. This web site, as well as the software described in it, is furnished for informational and instructional use only, is subject to change without notice, and should not be construed as a commitment by the instructor. The material presented at this site will contain examples of both good and bad practices and is presented “as is” for instructional purposes. The certified phlebotomy instructor (CPI) assumes all responsibility or liability for teaching the course. Phlebotomy Instructor Corp is simply an accreditation body that acts as an adviser to help medical professionals. Once applicants complete the teach the teacher course Phlebotomy Instructor Corp does not assume any responsibility and all responsibility falls upon the certified instructor. It is mandatory that certified phlebotomy instructors check all regulations with the proper States Board of Education.
Board Of Education Rules and Regulations
Teaching without approval from the Board of Education and/or other vocational school organization is a mandatory regulation in many states, thus removing all liabilities from Phlebotomy Instructor Corp. Phlebotomy Instructor Corp does not certify phlebotomy students but does provide a list of phlebotomy certification agencies which the phlebotomy instructors are responsible for. The refund policy is revoked after the applicant has access to phlebotomy education materials. Once the applicant has access to the platform its applicants responsibility to finish all information and pass the final exam in order to be certified, you will have 60 days to finish the materials. If applicants do not finish the program, the certification will not be issued, and you will forfeit your application fees.
